Anyone can make a referral to Future Care Support. This includes support coordinators, healthcare professionals, family members, friends or even the individual themselves. As long as you have consent and enough information to complete the form, we’re happy to receive it. If you’re unsure about any part of the process, just reach out. We’re here to help guide you through it and make sure the person being referred gets the support they need.

To make a referral, we’ll need some basic details about the person you’re referring, including their name, contact information, NDIS number (if available), and a brief overview of the type of support they’re looking for. The more details you can provide, the better we can prepare. Don’t worry if you’re unsure of some things, just include what you know and we’ll follow up with you or the participant if we need anything else.

Once we receive your referral, our team will review the information and get in touch as soon as possible. We’ll have a chat to understand the person’s needs and confirm any extra details. From there, we’ll guide them through the next steps, whether that’s an initial consultation, meet and greet or setting up services. Our goal is to keep things simple, clear and supportive right from the start.

Wait times can vary depending on the type of support needed and the person’s location, but we always do our best to respond quickly and get services started as soon as we can. If there is a short wait, we’ll keep you updated and let you know exactly where things are at.
